The Uniform Hazardous Waste Manifest is used to track hazardous waste from a generator’s site
to the site of its disposition. The form is prepared by ALL generators who transport,
or offer for transport, hazardous waste for off-site treatment, recycling, storage, or disposal.
Each party that handles the waste signs the manifest and retains a copy for themselves.
This ensures critical accountability in the transportation and disposal processes.
Once the waste reaches its destination, the receiving facility returns a signed copy of the
manifest to the generator, confirming that the waste has been received by the designated facility.
The Uniform Hazardous Waste Manifest is required by both DOT and EPA.
The Uniform Hazardous Waste Manifest form is a 6 part,
carbonless paper form that measures 8 1/2" by 11" after the stubs are detached.
Snap-Out Forms
The snap-out format has tear-off stubs across the top of the
form and can be filled out using a typewriter or by hand. Each pack contains 50 forms.
